You need to download a poll script cloud. It can't be done in HTML because HTML cannot process any input!!!

Google "free poll software php" or something, You'll find something simple enough..  :)

HTML and CSS just control how and what is displayed on screen, Nothing else. It's like wallpaper and paint... You still need a hammer and nails to do anything really special. PHP, Perl, Even Javascript process things and make things happen. HTML doesn't.
